We will only be providing the regular flu vaccine to age 18+ . The high dose flu vaccine is not available.

Open for: All residents of Jefferson Township 18 years and older (must provide proof of residency)  

Fee: $20.00. It is free for Medicare Part B members they have to provide their card.

When you arrive:

• Please remain in your car until you are called in by clinic staff.
• All persons must wear a facial covering that covers your mouth and nose.
• Please bring your filled out consent form the day of your appointment. (click here to view the consent form )
• Please only bring in the minimum number of persons needed for your appointment.
• If you are not feeling well or have tested laboratory confirmed positive for COVID-19, please do not attend your appointment – please call the health department to reschedule.
