Message from the State of New Jersey
The general public can now pre-register, and this will allow residents to input their personal, contact, health insurance, and demographic information. It will also allow for individuals to schedule an appointment for those who are immediately eligible based on screening questions.
We’ll ask some questions to determine when you are eligible to receive a vaccination.
Filling out this form should take about 15 minutes, and the information you enter will be kept private with the State of New Jersey.
“An exact time frame for the different phases cannot yet be determined,” said Donna Leusner, spokeswoman for the state’s Health Department. “Decisions about priority groups and how the doses will be spread across the state may change based on changes in vaccine supply and public demand.”
New Jersey hopes to vaccinate 70% of New Jersey adult residents — about 4.7 million people — in the next six months. Officials have said doses should be available for the general public by April or May.

- Name & Address Information
- Insurance information if any
- List of questions to help categorize you into the right group (i.e. Healthcare Worker)
It is expected that New Jersey will continue to use the predefined categories, with the purpose of this registration system to help the State call you when your group is up for the Vaccine Shots.
We highly recommend registering to ensure you are notified as soon as possible when it’s your turn.
